Optimize Patient Care & Compliance with
Care Management
Babyscripts operates as a clinical extension of the OB care team, providing real-time remote monitoring for disorders in pregnancy. As part of Babyscripts’ RPM model, every patient has access to a care manager. Care managers ensure that patients stay on track with their care plan—and that providers get accurate, timely data they can rely on.
About Babyscripts Care Managers
Who are our care managers?
Our care managers are RNs or licensed CNAs/MAs, who follow structured, evidence-informed protocols aligned with ACOG and SMFM guidelines. They hold state licensure and operate within their scope of practice while supervised by the ordering physician or midwife.
*All clinical decisions remain under the direction of the patient’s provider.
What do care managers do?
- Coach patients enrolled in high-risk BP monitoring
- Support device set-up and use
- Monitor patient-submitted BP readings
- Assess symptom severity
- Perform routine check-ins
- Educate the patient on risk & escalate to the OB team when appropriate
- Support broader care coordination needs (e.g., medication questions, unreported ED or L&D visits, social risks)
How is care management effective?
Babyscripts’ care management platform drives compliance to the code set, improves efficiencies, and enables proper documentation to prevent fraud, waste and abuse. Content and length of care manager interactions are documented in the Babyscripts care management platform as well as instances, timing, and severity of vital sign readings.

"The care manager did a great job making sure I knew what to expect using this program and monitoring my health during the remainder of my pregnancy and postpartum. I feel very comfortable using the machine. It gives me peace of mind knowing that if there is a concern the system will call me."
"On my own it's easy to procrastinate taking my blood pressure or ignoring symptoms. With the app it's an easy reminder, and the text and phone call check ins help me know when something is serious and I need to pay close attention. They also put me at ease with suggestions to reduce my systems.”
"The call was informative and personalized which was appreciated. She took time to ensure that our next meeting would fit into my schedule. I appreciate the opportunity to be able to keep track of my blood pressure. It helped to identify when I wasn't feeling well and got me in touch with my OB's office in a timely manner on a weekend."
"[The care manager] made sure my cuff was working properly, and made sure I knew how to use it. She also made sure I knew warning signs to look out for and asked if I had any questions. I feel better knowing that if a blood pressure issue were to arise, my provider would know about it quickly (versus not finding out until I have an appointment, especially now when there are four weeks between appointments)."
"Very understanding & the things I didn't understand, they took their time to listen & explain to the best of their knowledge. [The cuff] is easy to use. Someone is always there to help. They actually keep in touch & check on the status & well being of your baby."
"I feel more comfortable at home knowing that my blood pressures are continuously being monitored by trained staff."
"[My care manager] gave me pointers to help bring my blood pressure down and how to make sure I'm taking it correctly! She was very kind, helped me understand why it's important to keep monitoring it and what I need to watch out for! I felt better knowing someone was watching my blood pressure and I didn't have to be worried about calling to see if it was too high!"
"I am so glad that you told us to go to the hospital, we are so glad that we went. You guys saved me and my baby's life."
